CSA Report Form

This form is to be used by campus CSA’s to report crimes that fall under the Clery Act. Clery reportable crimes include: Criminal Homicide, Sexual Assault, Robbery, Aggravated Assault, Burglary, Motor Vehicle Theft, Arson, Liquor/Drug/Weapon Law Violations, Hate Crimes, and Violence Against Women. For more detailed descriptions of these crime please refer to the DCB Annual Safety and Fire Report.
